Simply put...our program works.  We give your hospice the tools needed to effectively manage your prescription program for your hospice patients.  Our program costs you nothing to participate, it is extremely easy to use, and there are no long-term contracts to sign.  All we do is work with Hospices and prescription benefits. How Palliative Drug Card Works… 

We offer robust plans centered around your needs.  A typical plan will work as follows: 

·      Upon new patient admission into your hospice, you simply fax or call the patient’s information to Palliative Drug Card.  If faxing, we offer a simple template for you.  PDC also has the ability to obtain patient eligibility electronically if you desire.

·      We take the patient information and enter or load it into our system.  Our system then generates a sheet that is faxed to the patient’s pharmacies. This fax notifies the pharmacy of the patient information and that the patient will be obtaining prescriptions through their pharmacy.  This process is all done within minutes of receiving the patient information. 

·      If your hospice elects, a member of our Pharmacy staff will perform a med-sheet review on each new patient and check for appropriate medication use, possible cost saving alternatives, and drug-to-drug interactions and precautions.  We fax the information provided by our Pharm.D. back to you for review.  We can also forward the med-sheet to the selected pharmacies limiting the number of faxes your staff needs to send. 

·      The processes you use to obtain the prescriptions at your local pharmacy WILL NOT CHANGE.  The only difference is the pharmacy submits the hospice patient’s drug information  online to Palliative Drug Card.  Instead of getting a bill by one or more of your local pharmacies, you get a simple detailed billing for each patient through Palliative Drug Card as well as your custom designed reporting catalog based on your needs.  It can not get any easier.
