Through intensive research and input from our hospice clients, pharmacists, and clinicians, PDC Rx has developed a practical formulary. This formulary outlines medications commonly used in hospice, as well as medications specific to each body system. PDC Rx understands and respects that prescribing habits and medication preferences can be unique to each hospice and its providers. Therefore, the PDC Rx formulary is dynamic and 100% customizable.
